Qutexes: Implement them and supporting classes

Implements: LockSet, SerializedAsynchronousContinuation,
	LockerAndInvoker, LockerAndInvokerBase, Qutex.

Very big leap in functionality here. See qutexes.md for
an explanation of what we've done.
